PREPARING FOR YOUR HOSPITAL VISIT

At Zuid-Afrikaans Hospital, we want your admission process to be as smooth and stress-free as possible. This section provides all the essential information you need before your admission.

Information needed for your medical aid

Your medical aid will require the following information from you:

The hospital’s name and practice number: Zuid-Afrikaans Hospital – Practice number: 5805112

The date of admission and procedure

ICD-10 codes (your diagnosis description) and the RPL codes (the description of the procedure you will undergo), which will be provided by your treating doctor

Kindly ensure that you obtain/confirm the following with your medical aid:

Your hospitalisation authorisation number

High cost medication which will require a letter of motivation from your treating doctor

Costs that are covered, the maximum benefits available, any exclusions, whether a co-payment is required on admission, and the prosthesis limit if applicable

What to bring on the day of admission

Please ensure you bring the following with you on the day of admission.

Identification

On the day of admission, bring your driver’s license, ID or passport, your medical aid card with authorisation number, and the names and contact details of your family or friends

Referring doctor’s name and contact number

Personal

Comfortable clothing or sleepwear, socks and slippers

Toiletries, such as toothbrush, toothpaste

Please do not bring any valuables to the hospital. The hospital does not take responsibility for the loss of any valuables

Assistive devices

Glasses, contact lenses and magnifying glasses

Walkers, crutches, hearing aids or dentures

Medical information

A list of previous hospitalisations, illnesses, operations, and allergies

Radiology images and reports, if applicable

Ensure you have a list of all medications you are using, including chronic medications, in their original packaging with dosages and schedules

Notes

Private patients will pay the full estimated amount as a deposit

Please note that this is only an estimate of cost and not the final account

This is payable on admission and can be paid by cash or credit card

To speed up the admission process, please complete the Patient Admission Form at home and forward the completed form to: 

admission@zah.co.za or use the pre-admission link on the website to complete the form

Valuables

We strongly recommend that you leave all your valuables at home. Zuid-Afrikaans Hospital does not take any responsibility for the loss of valuables.

You will only need essentials such as toiletries and sleepwear in your room. On transfer to High Care or ICU, you will only be allowed to take your toiletries with you.

WIFI

Patients are welcome to purchase data from reception which can be used whilst your are in hospital.
